1. St. Martin de Porres Hospital will not judge a patient by sex or sexual
orientation, race or color, age, occupation, national origin, etc, nor give
out different levels of treatment to different classes of people.
2. All staff members of St. Martin de Porres Hospital are required to wear a
badge or identification. If any person not wearing personal identification
approaches you to give assistance or medical attention, please do not
accept their assistance.
3. Insist on "patient oriented care" as a basis of care at St. Martin de
Porres Hospital. While admitted as an in-patient, our doctors may
report to you: your condition, medical examination, related
information, treatment plans; and provide care for you and/or inform
your family members.
4. If you have questions about the treatment or service given by our
medical team, we encourage you to ask your doctor or staff members
about the questions, when you do not understand or receive reasonable
explanations about your care. Upon your request, we will provide care in
a manner that protects your privacy and dignity, without informing friends,
relatives, or immediate family members, as so requested.
5. When the patient needs surgical treatment, St. Martin de Porres
Hospital will require the patient or his/her spouse, next to kin, or related
family, to sign an agreement - Consent Form to Perform a Specific
Surgery and Administer Anesthesia. Before signing it, the doctor will
explain the purpose of this practice, surgical success rate, and/or
possible syndromes that may be dangerous.
According to the medical code, in a situation of dire emergency and to
rescue the patient; first, aid the patient without the signature of
agreement. Otherwise we will proceed with the surgical procedure and
administration of anesthesia only with an agreement from the patient or
his/her spouse, next to kin, or related family.
6. St. Martin de Porres Hospital keeps all the personal health information
and treatment confidential. In case you do not want this information
given out to visitors (friends, family, etc.), please inform the proper
Hospital Staff and/or Administration.

7. St. Martin de Porres Hospital may inform the patient's medical condition to his/her next of kin. If your personal confidentiality is requested, please inform the proper Hospital Staff and/or Administration in advance.
8. In accordance to the doctor's diagnosis, if patient's illness is considered very serious and in a terminal stage, the hospital may provide the form: "In Advance Medical Treatment Refusal Notice". Patients may pre-specify someone to fill out this form as to who will make decisions and speak on his/her behalf, or the consent form: "Refusal of CPR", as a protection of the patient's medical rights and authority.
9. St. Martin de Porres Hospital is an acute care, academic and research hospital with programs that train outstanding faculty members, hence we do ask your complete cooperation while in association with the Medical Program. A patient does have the right to refuse any irrelevant procedure, examination, or other activities. Your rejection will not affect to any less degree, the treatment or service given you.
10. If any corporeal therapy is agreed upon during the time of being an in-patient, the hospital should clearly explain to the patient or someone who can make decisions and speak on your behalf, the following:
1- the purpose and method of the procedure;
2- possible side effects and risks;
3- anticipated result;
4- statement of other sources of treatment procedures available;
5- acceptance of the fact that the patient can change his/her mind, and refuse the original therapy agreement at any time, in accordance to Medical Law Code 57.
