The patient has to register and go to the doctor for diagnosis certificate. |

The patient could authorize others to request for a diagnosis certificate
by showing an authorized letter, and bring both the patient's and the
authorized person's IDs. |

| Labor Insurance, Farmer's Health Insurance Disability Certificate |
| 1. |
Bring all related documents, and request the certificate from doctor. |
| 2. |
We will send all documents to Bureau of National Labor Insurance.
Patient will be given a receipt to bring back to his/her organization. |

| The Major Illness/Injury Certificate |
The Patient can request the doctor to make a diagnosis certificate, then apply for The Major Illness/Injury Certificate to Bureau of National Health Insurance. |
| Birth Certificate |
Bring the ID cards of parents to window 9, the next day after birth. |
| Death Certificate |
Bring the deceased's ID card to window 9. |
| Duplication of Patient Medical Records and Medical Summary |
Bring patient's and family member's ID cards to the original doctor according to registration procedure. |

Doctor will authorize certificate only on authentic medical records. Please don't ask doctor to write
any inauthentic information on the medical certificate. |
